HPHS Online Self-Assessment Tool Introductory Passage:

The Health Promoting Hospitals Network was founded by the WHO in recognition of the importance of a settings approach for health promotion. A Health Promoting Hospital and Health Service is an organization that aims to improve the quality of health care, the relationship between health services and the community, and the satisfaction of patients and staff.

The Singapore HPHS framework is guided by the 5 WHO HPH standards which will help network members to direct health promotion initiatives, and monitor and evaluate their efforts.  The standards emphasize improving the health potential of individuals through the provision of information, counselling, training and community activities to improve health. They address not only patient care, but the health of staff, links to the community and organizational development.  

  1. Standard 1 addresses management policy.
  2. Standard 2 focuses on patient assessment. 
  3. Standard 3 describes patient information and intervention.  
  4. Standard 4 highlights what it means to create a healthy workplace.  
  5. Standard 5 emphasizes collaboration and partnership in the community.

The purpose of this assessment tool is to help your institution assess where you are in terms of the HPHS standards for health promotion.  Upon completion of the questionnaire, you will be provided with a report giving feedback on how well you performed on each of the five HPHS standards.  The report will provide suggestions for improvement in each standard to guide you on your journey to becoming a health promoting institution. You must print a copy of this report and the completed questionnaire for safekeeping.
